  • In the Medical Deductions section of your ITR12 you are required to complete information for each of the medical schemes of which you were the principal/main member during the year of assessment.

  • This means that if you were a member of more than one medical scheme during the year of assessment, you must provide details for each of the schemes as per the information on the medical tax certificates received from your medical schemes.
    • Indicate the number of medical schemes of which you were the principal/main member during the year of assessment:

    • Do not complete the field with code 4005 - “State the medical contributions made by yourself and/or your employer to this scheme”

    • Complete the field with code 4020 - “State any medical expenses paid by you that was claimed from your medical scheme and reflected on the medical certificate (other than physical impairment or disability expenses)” with the amount reflected on your medical scheme tax certificate received from your medical scheme.
Please make sure that you capture the correct contribution amounts under the correct Medical Scheme.
